  • Screenshot or screen recording: Include a screenshot or screen recording to visually demonstrate the issue, if possible.
  • Browser and version: Specify the browser (e.g., Google Chrome, Mozilla Firefox, Microsoft Edge, Apple Safari) and its version number in which the issue was encountered.
  • Operating system and version: Mention the operating system (e.g., Windows, macOS, Linux) and its version number used during the occurrence of the bug.
  • Device type: Indicate the type of device used (e.g., desktop, laptop, tablet, smartphone) and its model, if applicable.
  • URL: Provide the URL of the web page where the bug was encountered.
  • Date and time: Note the date and time when the issue was first observed.
  • Error messages: Include any error messages displayed by the web app, if applicable.
  • Console logs: Attach the browser console logs (if possible) to help developers identify any errors or warnings that might be related to the bug.
  • Frequency: Indicate how often the issue occurs (e.g., consistently, intermittently, only once).
  • User account: If the bug is related to a specific user account, provide the relevant account information, such as the username or user ID, without compromising the user's privacy.
  • Severity: Assess the severity of the bug (e.g., critical, major, minor) and its impact on the overall functionality of the web app.
